Holton Community Hospital (HCH) is pleased to announce a new chapter for its Home Health and Hospice services.

This decision comes after the hospital’s difficult announcement in September of the closure of its award-winning Home Health and Hospice department, effective December 31, 2024. That closure was necessitated by significant financial challenges brought on by changes in healthcare payment models, the expansion of Medicare replacement plans, and increasing competition from other agencies. Despite many years of subsidizing the department, the hospital could no longer sustain its operations.

Transition and Continuity of Care

Holton Community Hospital is collaborating closely with Faith Home Health and Hospice to ensure continuity of care for patients and families. A key priority is retaining the hospital’s dedicated local staff within the new structure, ensuring that patients continue to experience the same compassionate care they have come to trust. Faith Home Health and Hospice will also work directly with patients and referral sources to make this transition as seamless as possible.

Patients in need of Home Health services will be admitted on a case-by-case basis until the transition is complete. After November 17, 2024, Faith Home Health and Hospice will take over full management of services, ensuring uninterrupted access to quality care for those in need.
