October 15, 2024 – [Holton, Kansas] Holton Community Hospital has been chosen to participate in the Rural Healthcare Provider Transition Project (RHPTP). This initiative, supported by the Federal Office of Rural Health Policy (FORHP), aims to strengthen small rural hospitals and certified Rural Health Clinics (RHCs). The program focuses on key areas like efficiency, quality, patient experience, and safety.
Strengthening Rural Healthcare
The RHPTP is managed by the National Rural Health Resource Center. Its goal is to prepare small rural hospitals and RHCs to succeed in a healthcare system focused on value-based care. Holton Community Hospital will receive technical assistance through consultations with recognized experts. Each year, only five hospitals or RHCs are selected based on readiness and commitment. The selection process is competitive and looks for leadership dedicated to implementing best practices.
Commitment to Community Wellness
Beyond this project, Holton Community Hospital continually works to improve community health. Through initiatives like our Community Health Needs Assessment (CHNA), we identify and address local needs. Our motto, *“Personalized Care. Hometown Heart,”* represents our dedication to serving our neighbors, family, and friends. We take pride in building personal connections and delivering individualized care.
