The Jackson County Community Resource Guide is a centralized directory designed to help residents quickly find local assistance programs and community support services available throughout the county. The guide brings together information on organizations offering housing support, food assistance, transportation services, healthcare access, behavioral health resources, medical equipment programs, family services, and other forms of community assistance.

By organizing these resources in one place, the guide helps individuals, families, healthcare providers, and community organizations easily connect people with the support services available across Jackson County.

Connecting Residents With Local Resources

The online directory includes resources for housing assistance, food programs, transportation services, healthcare access, behavioral health support, medical equipment, family services, and other community resources available to residents of Jackson County, Kansas.

“This guide helps bring together many of the organizations working every day to support the health and well-being of Jackson County residents,” said Cody Utz, Senior Director of Quality and Informatics • Patient Care Services and Quality at Holton Community Hospital.

Serving Communities Across Jackson County

Resources included serve residents throughout Holton, Hoyt, Mayetta, Denison, Circleville, Netawaka, Soldier, Whiting, the Prairie Band Potawatomi Nation Reservation, and surrounding rural communities.

Supporting Community Health Needs

Holton Community Hospital developed the Jackson County Community Resource Guide as part of its ongoing commitment to improving community health and access to services. The guide supports priorities identified through the hospital’s Community Health Needs Assessment (CHNA), a process conducted every three years that brings together community leaders, healthcare providers, and residents to evaluate the health needs of Jackson County. The CHNA examines health outcomes, risk factors, and social determinants of health while helping guide strategies that strengthen local healthcare delivery and community support services.
