2024 Community Health Needs Assessment survey linksYour Voice, Our Mission: Shaping the Future of Healthcare in Our Community

Holton, KS – 9/5/2024 – Holton Community Hospital is inviting all community members to participate in its ongoing Community Health Needs Assessment (CHNA), a vital process that will shape the future of healthcare services in our area. Through the CHNA, the hospital seeks to identify and address the most pressing health needs of the community, ensuring that everyone receives the care they need.

What is a Community Health Needs Assessment?
A CHNA is a systematic process conducted every three years by non-profit hospitals like Holton Community Hospital. This process involves gathering data on various health outcomes, risk factors, and social determinants of health within the community. The hospital also seeks input from community members, public health experts, and local leaders to gain a comprehensive understanding of the community’s health needs. The insights gathered from the CHNA are used to develop strategies that address these needs, ultimately leading to improved health outcomes for the entire community.

Community Health Town Hall Meeting and Online Survey
To kick off the CHNA process, Holton Community Hospital has launched an **online survey** on September 4th, which is now available to everyone. The survey is designed to gather input from community members about the most significant health issues facing our area and how the hospital can better serve the community. Your feedback is crucial in guiding the hospital’s future efforts.

– Deadline: October 14th

Following the survey, Holton Community Hospital will host a Community Health Town Hall Meeting to discuss the survey results and gather additional input. The meeting will be an open forum for residents to learn more about the CHNA process, discuss significant health issues, and share their ideas on how the hospital can improve health services and resources.

– Date: November 7th, 2024
– Time: To be determined by survey results
– Location: The Courtyard Event Center, 426 Pennsylvania Ave, Holton, KS 66436

Why Participate?
By participating in the CHNA, community members have a direct impact on the healthcare services provided by Holton Community Hospital. The input gathered will help the hospital prioritize the most significant health issues, identify gaps in services, and develop strategies to address these challenges. This is a unique opportunity for residents to make their voices heard and contribute to the health and well-being of their community.

Your Voice, Our Mission
At Holton Community Hospital, we believe that the best way to serve our community is by listening to the people who live here. The CHNA is not just a requirement; it’s our commitment to understanding and meeting the health needs of our community. Your voice is essential in this process, and together, we can shape a healthier future for everyone.
