Senior Life Solutions

We are a Holton Community Hospital program, designed to meet the unique needs of individual’s typically 65 and older experiencing depression and/or anxiety related to life changes that are often associated with aging.

Our Senior Life Solutions Team

Included on our staff is:

  • Teri Kruse, RN the SLS program director, who is responsible for directing and coordinating the activities of program staff to ensure quality patient care and providing community education to create awareness of the program.
  • Tiffany Blevins, LMSW the SLS therapist, who is responsible for providing therapeutic services for patients utilizing group, individual, and family therapy sessions.  She focuses on quality patient care and provision of services ordered by a physician. 
  • Jordan Carlson, the SLS Office Patient Coordinator, who is responsible for providing quality administrative and clerical services for the program staff and assisting our patients with care needs.   

Our Services

Following an individual assessment, patients meet up to three times per week in a supportive, encouraging group setting.  Services offered by Holton Community Hospital Senior Life Solutions include: A confidential, comprehensive assessment, group and individual therapy, family therapy, medication education and management, and after-care planning, where we continue to engage and support our participants. Even though this is a group setting, each  person is addressed individually based on their unique needs. 

Group Therapy

 At Senior Life Solutions, our group members, typically 7-10 people, receive group therapy to help work towards a better quality of life and mental health and well-being.  Group members help each other by sharing personal experiences, as well as their strengths, which can boost self-esteem and confidence. By interacting with other members and receiving feedback from the group and the therapist, each member of the group gains a greater  understanding of him or herself and has the opportunity to create relationships with other people who may be facing similar challenges.  During group therapy, the group may engage in open conversation guided by the therapist, which allows the group to support each other and share common experiences.  Often in group therapy, participants learn new skills to support their mental health wellness for the remainder of their lives.

Eligibility and referral process for Senior Life Solutions

A Senior Life Solutions clinician can provide a confidential assessment to determine a person’s eligibility for the program. Anyone can make a referral to our program, including self-referrals, primary care physicians, specialists, family members, friends, community groups, and nursing or residential facilities.

We are committed to providing excellent care and being invested in our community.  We are always available to answer questions and to provide educational in-services and information to community members.  We also educate other healthcare professionals about mental health wellness and the specific challenges of aging.
