Patient Care Coordinators (Case Management)

We provide hospital case management, utilization review and discharge planning to assure that our patient’s progress through the continuum of care and are discharged to the least restrictive environment. We accomplish this by coordinating with the patient, patient families/caregivers, Healthcare providers, durable medical equipment providers, other hospitals, nursing facilities, home health, hospice, medical clinics and other community resources.
