Why Community Support is Needed

For 87 years, Holton Community Hospital has provided trusted healthcare to Jackson County. In 1999 Rural Health Resources of Jackson County was formed as a 501(c)(3), took ownership of Holton Community Hospital and has operated without any local tax support. But today, rising costs and changes in insurance reimbursement make it harder than ever for rural hospitals like ours to keep quality care close to home.

Over the past four years, operating expenses have increased by 48%, while labor costs alone have risen 28% as we work to recruit and retain top talent in a competitive market. At the same time, more of our patients now use Medicare Advantage plans, which reimburse hospitals significantly less for the same services.

Unlike Holton, other nearby critical access hospitals — including Onaga, Sabetha, Marysville, and Wamego — already receive local tax funding to help keep their doors open. By approving this modest 0.25% Sales Tax, Jackson County can ensure that Holton Community Hospital continues to deliver essential services like emergency care, surgery, imaging, cardiac care, and family medicine right here at home.

Without this sales tax funding, we will have to make difficult decisions on services being offered, meaning more families will have to travel farther for healthcare, and local jobs will be at risk. With your support, we can keep our hospital strong for generations to come.

What Does “Critical Access Hospital” Mean?
“Holton Community Hospital is designated as a Critical Access Hospital (CAH) — part of a special rural healthcare program created to make sure smaller, less populated, remote communities have essential medical care close to home. As a CAH, we’re required to keep 24/7 emergency services available, but we serve fewer patients than larger urban hospitals. This unique model helps us provide vital services here in Jackson County — but it also means community support is key to staying strong for the future.”

Every year, thousands of patients rely on us for lifesaving and life-improving care, right here at home. In Fiscal Year 2024 alone (April 2023–March 2024), our team provided:

  • 3,894 Emergency Room visits — ensuring urgent care is always close by when you need it most.

  • Over 7,800 Radiology visits — delivering advanced imaging services locally.

  • 5,497 Specialty Clinic visits — so our community can see trusted specialists without traveling far.

  • 14,911 Family Clinic visits — keeping families healthy with primary and preventative care.

  • 4,372 Cardiopulmonary Rehab visits — helping patients recover and manage heart and lung conditions.

  • 42,974 Laboratory tests — supporting accurate diagnosis and treatment plans.

  • 573 Surgeries — providing safe surgical care close to home.

  • Over 18,000 Rehabilitation visits — from physical to occupational therapy, helping patients heal and return to their daily lives.

And this is only a portion of the vital services we offer every day. By keeping these services local, we reduce the burden on families who would otherwise travel far for care — saving time, money, and providing peace of mind when it matters most.

Your Community Support helps us protect and grow this impact for everyone in Jackson County.

HCH's Economic Impact on Jackson County

Holton Community Hospital is not only essential for local healthcare — it’s one of Jackson County’s largest employers and a critical part of our community’s economic stability. According to the 2023 Economic Impact Report by the University of Kansas Institute for Policy & Social Research and the Kansas Hospital Association, Holton Community Hospital alone supports over 271 local jobs and contributes more than $18.4 million in annual labor income directly to Jackson County families.

When patients can get the care they need close to home, they spend their dollars locally — helping other small businesses, restaurants, and shops thrive. Local healthcare access also means our neighbors don’t have to take as much time off work or spend extra money traveling for care, which keeps more of our community’s income right here in Jackson County.

The proposed 0.25% Sales Tax will help keep these jobs in our community, support local spending, and protect the services that help families, farmers, and businesses stay healthy and productive. It’s an investment that multiplies its benefit many times over — strengthening our local economy while keeping quality healthcare local.

How the Community Support Dollars Will Be Used

If passed, Jackson County, Kansas would be authorized to impose a one-quarter percent (.25%) countywide retailers’ sales tax, the proceeds of which will be used to support hospital services in Jackson County, Kansas by Rural Health Resources of Jackson Co., Inc. doing business as Holton Community Hospital to which such proceeds shall be paid. The collection of such sales tax will commence on January 1, 2026, and terminate ten (10) years thereafter, all pursuant to the provisions of K.S.A. 12-187 et seq.

Every dollar from the proposed 0.25% Sales Tax will stay right here in Jackson County — dedicated solely to supporting Holton Community Hospital and keeping high-quality care local.

  • Maintain vital services: Ensure critical services like emergency care, surgery, imaging, cardiac care, family medicine, and mental health programs remain available close to home.
  • Invest in medical equipment and technology: Keep up with rising equipment costs and ensure our hospital has the modern tools needed to diagnose, treat, and care for our patients safely and effectively.
  • Protect financial stability: Offset the growing gap between operating expenses and declining reimbursements from insurance programs like Medicare Advantage — ensuring we can continue to operate without cutting services or closing departments.

If approved, the sales tax will last for 10 years, after which Jackson County voters will have the opportunity to decide whether to renew it. Holton Community Hospital is committed to full transparency: Annual financial reports will be shared with the public.

Together, we can protect local healthcare, invest in our community’s future, and keep quality care right here in Jackson County.

What Happens If Community Support Falls Short

Without the additional funding from this modest 0.25% Sales Tax, Holton Community Hospital will face difficult choices that could affect every family in Jackson County.

Rising costs and lower insurance reimbursements already stretch our resources thin. If we do not secure local funding support, we risk:

  • Reducing local services: Some specialized services could be scaled back, forcing patients to travel farther for care.

  • Longer wait times: With fewer resources, staffing shortages could grow, making it harder to see your local providers when you need them.

  • Lost local jobs: Healthcare is one of our community’s largest employers — every cut means jobs, income, and local spending could be lost.

  • Weaker local economy: When patients travel elsewhere for care, their spending leaves with them, hurting small businesses and our county’s tax base.

Many nearby rural hospitals already rely on local tax support to keep their doors open. This Sales Tax is our community’s way to make sure Holton Community Hospital stays strong and keeps high-quality care close to home — not just for today, but for generations to come.

Frequently Asked Questions (FAQs)

A: As a Critical Access Hospital and a Level IV Trauma Center, part of our purpose is to stabilize the sickest patients and transfer those who require highly specialized care. But here’s the truth:
👉 Only 6% of patients are transferred — meaning 94% receive their care right here in Holton. That’s an incredibly low transfer rate compared to many rural hospitals.

A: No — Holton Community Hospital is committed to staying open and serving Jackson County. However, without local funding support from this Sales Tax, we may face tough choices that could include reducing services, delaying equipment upgrades, or deferring maintenance. This Sales Tax helps ensure we can keep care local, maintain our facilities, and continue serving our community for years to come.

A: For 87 years, Holton Community Hospital has proudly served Jackson County with no local tax support. But today, the rising costs of materials, maintenance, and labor — combined with lower insurance reimbursements, reductions in Medicaid, and the gaps left by some Medicare Advantage plans — make it harder to keep all our services local. Your Community Support will help us maintain essential care, invest in updated equipment and facility improvements, and keep high-quality healthcare close to home for everyone in our community.

A: A Sales Tax is fairer because everyone who shops in Jackson County contributes — including visitors. According to KDOT an average of about 9000 vehicles pass through Jackson County each day. This spreads the responsibility across our whole community instead of putting all the burden on local homeowners and farmers.

A: For every $100 you spend on taxable goods in Jackson County, you would pay just 25 cents more. As of January 1, 2025, Kansas has removed the state sales tax on groceries and food ingredients meant for home consumption — but local sales taxes, like this one, still apply. Prepared foods, restaurant meals, and non-food items will continue to be taxed at both the state and local level.

A: No — this is a common misconception. The building expansion project was responsibly funded through a $14.6 million loan offered by the USDA’s Rural Development agency, along with more than $2 million raised by the community through our nearly year-long “Embrace the Future” campaign, which began in November 2018. Far from hurting our finances, this investment made it possible to grow services that generate revenue, including expanded Outpatient Clinics, Cardiac-Pulmonary Rehab, and Radiology. The new building also houses the Holton Family Clinic, administration offices, medical records, our business office, Senior Life Solutions, and a community meeting room that’s used daily for staff meetings and public education. This expansion strengthened our ability to serve Jackson County — and your Community Support today helps maintain and build on that growth for the future.

A: No — in fact, the opposite is true. Under our current leadership over the last 10 years, Holton Community Hospital has doubled its annual revenue streams, growing from $22 million a year to $44 million in 2024. Like all Critical Access Hospitals, our business model is unique: we’re required to stay open 24/7 with emergency services, but we face the same rising costs and insurance challenges as larger hospitals — just with fewer patients to help cover those costs. That’s why expanding outpatient services, clinics, and specialty care has been so important to our financial stability. Your Community Support helps us continue to grow these services and protect local healthcare for everyone in Jackson County.

A: one-quarter percent (.25%) countywide retailers’ sales tax, the proceeds of which will be used to support hospital services in Jackson County, Kansas by Rural Health Resources of Jackson Co., Inc. doing business as Holton Community Hospital to which such proceeds shall be paid. Funds will help maintain vital services like emergency care, surgery, imaging, and mental health programs — and will be used for critical equipment costs, technology upgrades, and to ensure safe, modern care for our community.

A: If approved, this Sales Tax will be in place for 10 years, at which point Jackson County voters will have the chance to renew it.
